Why adult ADHD is missed so often
The diagnostic picture was written from watching children. Most adults stopped matching it years ago — which is why so many people reach forty before anyone asks the question.
The picture was drawn from children
The criteria clinicians use were built by observing school-age children, mostly boys, in classrooms. A child who cannot stay in a chair is visible to a teacher within a week. An adult who has spent thirty years building a life around the same difficulty is not visible to anyone — including, very often, themselves.
What survives into adulthood is rarely the running and climbing. It is the inability to start a task you care about, the conversation you lose halfway through, the third coffee before a meeting that everyone else finds unremarkable.
Coping looks like competence
Adults who reach a consultation have usually built elaborate systems: alarms stacked on alarms, jobs chosen for their deadlines, partners who quietly handle the paperwork. From outside, the systems read as an organised person.
